To Become a New Member:

Option 1. Page one of the Work Order can be filled out and emailed to asregistry@gmail.com and then by calling them with card information. Do NOT email card information as email is not encrypted.

Option 2. Page one of the Work Order can be filled out and mailed with a check to the address on the form.
